Resident Users
A valid NYS Drivers License is required. Payment options include cash, bank check, money order, or a check that matches the name and signature on the drivers license. Individuals without a valid NYS Drivers License may complete the Out of State Individual Application Package detailed below.

Non Resident Users
Complete the Non Resident Individual Application Package – No Credit (PDF - 33kb) and you will receive one (1) CVR card to use at any MOSA transfer station. Payment options include cash, bank check, money order, or a check that matches the name on the application.

Small Business Customers – No Credit
Complete the Small Business Application Package – No Credit (PDF - 34kb) and you will receive one (1) CVR card to use at any MOSA transfer station. Payment options include cash, bank check, money order, or a check that matches the name on the application.

Customer Application – No Credit
Complete the Customer Application Package – No Credit (PDF - 37kb) and you will receive a CVR card for each one of the trucks listed on the application to use at any MOSA transfer station. Payment options include cash, bank check, money order, or a check that matches the name on the application.

Customer Application – Credit
Anyone interested in obtaining credit must complete the Customer/Credit Application Package (PDF - 45kb). You will receive a CVR card for each one of the trucks listed on the application to use at any MOSA transfer station.

Municipal Customers – Credit
A municipality interested in obtaining credit must complete the Municipal Customer Application Package (PDF - 39kb). You will receive a CVR card for each one of the trucks listed on the application to use at any MOSA transfer station.
